No need to change your Modem, the problem is with your Line Parameters. Just simply observe the Attenuation which is 40 and it is considered high(close to very Bad). Try changing your DSL Modulation from ADSL2+ to G.Dmt and then share a screen shot in order to check your Line Parameters as well as SNR Margin. And if I'm not wrong you must be on copper. A wise advise to you would be to shift your Line to Fiber (aka ONU) in order to get stable speeds,pings and nearly no disconnections what so ever. But for now do what I have advised and share its Screen Shot in order to see the problem.
