Immediate help regarding Photoshop!

Okay guys. As the title says, i really need help regarding Photoshop. I have attached this image:

I want any of you, to please, make this in 3d for me in photoshop(from scratch) using the channel feature as well. I would be really thankful. The measurements must be:

length: 12 inches
width: 18 inches
resolution: 300 pixels/inch
