As far as I know, its a DRM (anti-piracy) protection issue.
You will need to download a proper crack or follow instructions here:
Serious Sam 3 BFE rotation mouse DRM Protection v2 fix bug mouse - YouTube
Create:
Steam\steamapps\common
Then throw your SS3 BFE folder in it. Load the last save and it will work.
D:\Steam\steamapps\common\Serious Sam 3, doesnt work for me

Write clearly the level and place you are experiencing the 'code guard'.

Most important question. Which release and crack are you using?.
... and its not a bug, just so you know. Its an anti-piracy measure installed in the game by Croteam.
You can press '~' and see the red line with code guard activated message.
