Either PSU problem, or that graphics card has gotten too hot that its about to die.

Have you cleaned the heatsink of the graphics card using a blower?

Also mention your complete system specifications and the version of Windows you are running, and also the driver version of your AMD card (by going into AMD Catalyst).

Specs seem good, though CM eXtreme 600W seem to give some people some problems. I too am using this PSU and it gives strange garbled display after loadshedding turns my PC off unexpectedly. But it never gave problems on any game, despite my 6950 demanding more Watts and Amperes than 4870.

Okay now you have to install a software called "HWMonitor". Download it from the net.

Before starting a game, start this software. Then play a game for 2 or 3 minutes. Shut down the game and notice the minimum and maximum temperatures of your Processor and Graphics Card on HWMonitor.

      Post them here.

do you have the "cooler master 600w extreme series" power supply???

if you do.......that ps has a problem running 4870's........I suggest you get another brand of ps preferably.

I fixed a similar issue with my 4870 by underclocking it (the idea being that it was overheating/some load issue). Download Rivatuner or just use catalyst software to underclock both clocks by around 100mhz or more. Underclocking should be done at your own risk since well, it can damage your card further if you dont know what your doing.
p.s. also use rivatuner to raise the speed of your fan, again, at your own risk because some blogs say you can blow out your fan that way (mine blew out a year ago and now i just have a case fan next to it).
it sounds quite horrible but at the end of the day all my games work without issue.

Bro, Post a GPU-Z Screenshot of both tabs, Graphic Card and Sensors!
Then we'll be able to understand what really is your problem, your temps, psu etc.
